Abstract

Numerous cases of REM sleep behavior disorder (RBD) have been reported to be associated with other conditions such as neurodegenerative diseases, structural lesions in brainstem, sleep disorders, and medications. Particularly, Parkinsonism have the strong relationship with RBD. Most of structural lesions were located in brainstem tegmentum. RBD frequently coincides with narcolepsy. Taking serotoninergic or noradrenergic agents, and withdrawing of alcohol or several sedative drugs tends to provoke RBD. Many patients who are initially diagnosed with idiopathic RBD have neuropsychologic abnormality and eventually develop neurodegenerative diseases. These findings implicate that RBD is the neurologic disease, of which the pathophysiologic substrate is the brainstem tegmentum and multiple neurochemical system.
